These Terms & Conditions (“Terms”, "Terms and Conditions") form a legally binding agreement between you (“User,” “you,” or “your”) and YURA PLLC (“Company,” “YURÁ,” “we,” “us,” or “our”) governing your access to and use of the YURÁ website (the “Site”). By accessing or using the Site, you acknowledge that you have read, understood, and agree to be bound by these Terms & Conditions. If you do not agree, you must immediately discontinue use of the Site. These Terms apply only to use of the Site and do not replace any separate informed consent documents, HIPAA notices, financial policies, or clinical agreements required for in-person or TeleHealth services.
PURPOSE OF THIS SITE
The Site is provided for general informational and educational purposes related to YURÁ’s brand, philosophy, and services. Nothing on this Site constitutes medical, clinical, psychological, legal, financial, or other professional advice. All content is general in nature and is not intended to diagnose, treat, cure, or prevent any condition. Use of this Site does not create any provider-patient, professional-client, or advisory relationship between you and YURÁ. Such a relationship is only formed through execution of appropriate intake, consent, and service-specific documentation. Site content may include educational materials such as testimonials, case examples, or general outcomes information for illustrative purposes only. Individual results vary and are not guaranteed.
NO RELIANCE ON INFORMATION
We do not warrant that Site content is accurate, complete, current, or error-free. Content may be updated or removed at any time without notice. Any reliance on Site content is strictly at your own risk. YURÁ disclaims all liability arising from reliance on information provided on the Site.
ELIGIBLE USE
You must be at least 18 years old or have legal guardian supervision to use this Site.

HEALTH INFORMATION & HIPAA NOTICE
YURÁ may be subject to federal and state health privacy laws, including HIPAA, where applicable to clinical services. Information submitted through the Site (including contact forms, emails, or general inquiries) is not guaranteed to be secure and should not include sensitive health information. Protected Health Information (PHI) submitted through clinical systems is governed exclusively by our Notice of Privacy Practices and not by this document. By using this Site, you acknowledge that electronic communications may not be fully secure.

Content on this Site is provided for informational and educational purposes only and is not intended to constitute medical advice, diagnosis, treatment, or individualized clinical recommendations. No provider–patient relationship is created through website use, browsing, form submissions, communications, or introductory consultations. The purchase of supplements, products, or services through this Site does not establish a provider–patient relationship or constitute medical evaluation, diagnosis, or treatment. SERVICES, LICENSING, AND TELEHEALTH
YURÁ provides in-person services in Arizona. Telehealth services may be offered only where legally permitted and only when the provider is licensed in the state where the patient is physically located at the time of service and subject to applicable regulatory requirements. We reserve the right to decline or discontinue telehealth or in-person services based on clinical judgment, legal requirements, or operational limitations. Service descriptions on this Site are not guarantees of availability or outcomes. YURÁ does not bill or accept insurance for services provided.
ELIGIBILITY OF SERVICES
YURÁ does not provide services to individuals under the age of 18. All patients must be at least 18 years of age to schedule appointments or receive care. Patients receiving TeleHealth services must be physically located in Arizona or Colorado at the time of service. Eligibility for care is determined based on the patient’s physical location at the time services are rendered. Appointments scheduled for individuals under 18 or for patients who do not meet applicable location requirements may be canceled at the discretion of YURÁ. If an individual under 18 attempts to schedule or receive services, the appointment will be canceled. All services, evaluations, and treatment recommendations are subject to provider discretion, clinical appropriateness, and applicable licensing requirements. YURÁ and its providers reserve the right to decline, discontinue, or limit services when deemed clinically inappropriate or otherwise not medically appropriate.
INTRODUCTORY COMMUNICATIONS
Introductory consultations, if offered, are informational only and do not include diagnosis, treatment, or establishment of a provider-patient relationship. Participation does not guarantee eligibility for services.
FINANCIAL POLICY
Patients are responsible for reviewing and understanding all financial policies prior to scheduling or receiving services at YURÁ. By scheduling an appointment or purchasing any service, package, or membership, the patient acknowledges and agrees to the terms outlined in this Financial Policy. Patients agree that payment for scheduled appointments is required prior to the time of service. For appointments scheduled more than 24 hours in advance, full payment is due no later than 24 hours prior to the scheduled appointment time unless other arrangements have been made in advance and approved by the clinic. For same-day bookings or appointments scheduled within 24 hours of the appointment time, payment is due in full at the time of booking or prior to being seen.
Appointments require at least 72 hours’ notice for cancellation. Cancellations made with less than 72 hours’ notice, as well as missed appointments (no-shows), may be charged in accordance with the applicable cancellation policy disclosed at the time of booking. Rescheduling requests made within this timeframe are treated as late cancellations unless otherwise specified in writing. Memberships, packages, and prepaid services are subject to separate terms disclosed at the time of purchase, including cancellation, refund, and usage limitations. Services included within memberships or packages are only valid when used in accordance with all applicable clinic policies. Any additional services provided during an appointment, including but not limited to procedures, testing, reports, letters, or extended clinical services, will be reviewed and itemized at the conclusion of the visit and must be paid in full at checkout.
Patients are responsible for the full cost of all services rendered, regardless of the method of payment used, including but not limited to Health Savings Account (HSA) or Flexible Spending Account (FSA) cards. Use of HSA/FSA or any other third-party payment method does not alter or limit the patient’s financial responsibility for services provided. Any determination of eligibility, reimbursement, or coverage under such accounts is the sole responsibility of the patient. YURÁ does not bill insurance. Superbills may be provided upon request; however, reimbursement is not guaranteed and remains the patient’s responsibility.
By scheduling an appointment or purchasing services, the patient authorizes YURÁ to charge the selected payment method for all applicable fees in accordance with this Financial Policy and any other applicable Terms & Conditions or clinic policies.
PACKAGES AND MEMBERSHIP POLICY
YURÁ offers memberships and package programs ("Metabolic Mastery Series","MMS") designed to support ongoing wellness and provide cost savings for patients seeking regular care. Memberships and packages may include benefits such as discounted visits, laboratory services, in-office supplements, or other integrative therapies, as specified at the time of purchase. Benefits are valid only for services provided within YURÁ’s scope of practice and apply to authorized treatments, whether delivered via TeleHealth or in-person care. Telehealth services are available only to patients who are physically located in, and are a resident of, Arizona or Colorado at the time of service. In-person services are provided in Arizona and require the patient to be physically present in Arizona at the time of the visit. Eligibility for all services is determined based on the patient’s physical location at the time of care.
Packages and memberships are non-refundable for services that cannot be rendered due to failure to meet these eligibility requirements, including TeleHealth or in-person location requirements. Memberships may renew automatically unless otherwise specified, and package expiration dates, visit limits, or usage terms will be communicated at the time of purchase. All fees are non-refundable unless required by law. By enrolling in a membership or purchasing a package, patients authorize YURÁ to charge their selected payment method for the associated costs, and acknowledge that they have read, understood, and agreed to the cancellation, refund, and usage policies outlined in this agreement. Patients are responsible for reviewing their package or membership details and ensuring they use services in accordance with clinic policies. Questions regarding benefits, pricing, or expiration should be addressed prior to enrollment.
For package and membership-based appointments canceled with 72 hours or more notice will retain their service credit and may be rescheduled without penalty. Cancellations made within 72 hours of the scheduled appointment time, as well as no-shows, will result in forfeiture of the associated service, which will be deducted from membership or package benefits. Package and memberships purchases are non-transferable and may not be redeemed for cash. Purchases do not guarantee appointment availability, provider assignment, or treatment outcomes. Unused services do not automatically qualify for refunds, credits, or extensions unless required by law or expressly stated in writing within a separate financial policy. All memberships and packages require acknowledgment of the Terms & Conditions, Privacy Policy, and clinic Policies prior to enrollment or purchase.
YURÁ GIFT CARD POLICY
YURÁ Gift Cards (“Gift Card”, "Gift Cards") provide prepaid credit that may be applied toward eligible professional services offered by YURÁ. By purchasing, accepting, or redeeming a YURÁ Gift Card, the purchaser and/or recipient acknowledges and agrees to be bound by this Gift Card Policy, as well as YURÁ’s Terms & Conditions and other applicable practice policies. This YURÁ Gift Card Policy applies to all YURÁ Gift Cards, including physical and electronic (eGift) cards. YURÁ Gift Cards may be redeemed exclusively for professional services provided by YURÁ practitioners. Gift Cards are not valid for the purchase of retail products, supplements, or merchandise, unless otherwise expressly stated in writing by YURÁ.
The value of the Gift Card will be applied to the recipient’s account at the time services are rendered. If the cost of services exceeds the available Gift Card balance, the remaining balance must be paid by the patient at the time of service in accordance with YURÁ’s standard payment policies. Any unused portion of the Gift Card will remain as account credit for future eligible services. Gift Cards are non-refundable and may not be redeemed for cash or exchanged for monetary value, except where required by applicable law. Gift Cards are intended solely for personal use toward eligible services provided by YURÁ. When Gift Cards are used to book services, the same policies apply as standard appointments. Cancellations made more than 72 hours prior to the scheduled appointment will receive a 50% credit of the service purchased applied to the patient’s account, which may be used only toward eligible YURÁ services and is not redeemable for cash or transferable to other credit types. Cancellations made within 72 hours of the scheduled appointment time, as well as no-shows, will result in forfeiture of the full service value. Gift Cards may be gifted to another individual prior to redemption unless otherwise specified at the time of purchase. Once a Gift Card has been applied to a patient account or partially redeemed, the remaining balance may become associated with that account and may not be reassigned without the discretion and authorization of YURÁ. YURÁ Gift Cards do not expire unless an expiration date or promotional condition is explicitly stated at the time of purchase or required by applicable law. Any promotional, bonus, or discounted value associated with a Gift Card may carry separate terms or expiration conditions if disclosed at issuance.
YURÁ is not responsible for lost, stolen, damaged, or unauthorized use of Gift Cards, including unauthorized access to digital gift card codes or delivery information. The purchaser and/or recipient is responsible for safeguarding Gift Card information. The purchase, possession, or use of a YURÁ Gift Card does not guarantee service availability and does not obligate YURÁ to provide any particular service. All services remain subject to practitioner evaluation, professional appropriateness, scheduling availability, and applicable clinical standards. The purchase or possession of a Gift Card does not establish a provider-patient relationship with YURÁ or its practitioners. A provider-patient relationship is established only after appropriate intake, evaluation, and acceptance by a licensed provider in accordance with applicable laws and professional standards. Gift Cards may not be resold, marketed, or distributed for commercial or promotional purposes without the prior written consent of YURÁ. YURÁ reserves the right to modify, update, or amend this Gift Card Policy at any time, consistent with applicable laws and regulations. Any modifications will apply to future purchases and uses following publication of the updated terms.
COMMUNICATIONS
You agree that any information submitted through the Site is non-confidential unless submitted through designated secure clinical systems. Do not transmit protected health information through unsecured forms or email. By providing your phone number and/or email address, you consent to being contacted by YURÁ regarding appointments, scheduling, account matters, and other practice-related communications. YURÁ is not responsible for unauthorized access to information transmitted outside secure systems.
INTELLECTUAL PROPERTY
All content on the Site, including branding, design, text, graphics, logos, images, and media, is owned or licensed by YURÁ and protected by applicable intellectual property laws. No content may be copied, reproduced, distributed, or used for commercial purposes without prior written consent.
THIRD-PARTY SERVICES
The Site may contain links to third-party tools or services. YURÁ does not control and is not responsible for third-party content, policies, or practices. Use of third-party services is at your own risk.
DISCLAIMER OF WARRANTIES
The Site is provided on an “as is” and “as available” basis.
To the maximum extent permitted by law, YURÁ disclaims all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement.
LIMITATION OF LIABILITY
To the maximum extent permitted by law, YURÁ shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising from your use of the Site. Total liability, if any, shall not exceed the greater of: (a) $100 USD, or (b) the amount paid by you to YURÁ in the 12 months prior to the claim.
This limitation does not apply to liability that cannot be excluded under applicable law, including certain medical malpractice claims where legally required.
DISPUTE RESOLUTION & ARBITRATION
Any dispute arising from these Terms or use of the Site shall be resolved through binding arbitration in Arizona, except where prohibited by law. You agree to waive participation in class actions or class-wide arbitration. Either party may bring individual claims in small claims court if eligible. This clause applies to website-related disputes and does not override statutory rights related to clinical care where applicable law prohibits arbitration limitations.
INDEMNIFICATION
You agree to indemnify and hold harmless YURÁ, its owners, employees, contractors, and affiliates from any claims arising from your misuse of the Site or violation of these Terms.
FORCE MAJEURE
We are not liable for delays or failure caused by events beyond our reasonable control, including natural disasters, technical failures, government actions, or labor disruptions.
MODIFICATIONS
We may update these Terms at any time. Continued use of the Site after changes are posted constitutes acceptance of the updated Terms.
SEVERABILITY
If any provision is found unenforceable, the remaining provisions remain in full force and effect.
GOVERNING LAW
These Terms are governed by the laws of the State of Arizona, without regard to conflict of law principles.
